  • Diploma & Transcript of Records with Related Learning Experience
  • Updated PRC License
  • Board Rating
  • Board Certificate
  • Employment Certificate/s (previous to latest)
  • 2 Nursing Recommendation Letters from Nursing Supervisors/ Superiors
  • Training & Seminar Certificates
  • Passport
  • Birth Certificate
  • 4 pcs 2x2 or passport size white background pictures
  • OMA Certificate for Muslim applicant
For Ex-Saudi:
  • Fina exit stamp on passport or Electronic final exit visa copy
  • Declaration of final exit with no re-entry visa
